Working with a server that doesn't have one, and frankly, I don't know what you name the file (or where you put it) to work as one.

For what web server? Here's what I know for the two most likely options...
In IIS, they're defined through the properties of the server or directory objects. With apache, they're defined in the httpd.conf on server-wide, virtual server-wide, or directory levels.
