Do these words go together?

by chance do you have a policy number in reference
I am fairly certain that my English is understanding well, but words in quote go together not. Right?

been reviewing calls today and this person keeps using that phrase...my forehead might have a mark from hitting the desk a few times.

Replace IN with TO and it makes sense. But as is, no.

When the audit is finished, he will be informed of a "few" things
I think the most painful part is having to hear him repeat it 2-3 times per call when customers don't understand what they are being asked.

One of my first real jobs I had one of my responsibilities included reviewing technical support calls. It's hard when you hear people making the same verbal (or/or technical) mistakes OVER AND OVER.
I didn't mind correcting people's troubleshooting/technical mistakes (or asking them why they did it that way... maybe they were onto something I wasn't), but correcting language usage is always a tough one because it's REALLY hard to do it without sounding like a pedantic jerk, you have to concentrate on WHY it needs corrected (it confuses the other person etc) and not just because it's incorrect.
